This Ohio Blanket Sales Tax Exemption Form is provided for Á½ÐÔÉ«ÎçÒ¹ University employees who need to provide a sales tax exemption to a vendor for purchases by Á½ÐÔÉ«ÎçÒ¹ University. Please review Sales Tax Exemption Information prior to completing the exemption certificate.

If you are a vendor that requires a sales tax exemption certificate, please contact the Á½ÐÔÉ«ÎçÒ¹ University department that is making the purchase to obtain a signed form.

Instructions:

Please add the Vendor Name, Department address, sign and date the form.  Anyone authorized to make purchases with Á½ÐÔÉ«ÎçÒ¹ funds may sign the form.
