Center researchers, led by Edward Chiyaka and John Hoornbeek, recently published a study that evaluated a program targeting chronic illnesses using the Pathways Community HUB care Coordination model.
The article, published in Ohio Journal of Public Health, focused on documenting the development, challenges and management responses, and lessons learned from implementing the Pathways Community HUB care coordination program in Toledo, Ohio.
New HUBs are being implemented in Ohio, and elsewhere, and this paper comes at the right time as it details some of the challenges faced when implementing such a program, as well as potential programmatic actions that can be taken to deal with such issues. The article also describes the program鈥檚 implications for public health practice and policymakers, particularly in Ohio.
